a56f1ca1ed Cleanup MV/MD macros a little.
When using variadic macros there's no need for IF_MD2/IF_MV2 to deal
with function parameters. IF_MD/IF_MV are enough.

Throw in IF_MD_DRV/ID_MV_VOL that return the parameter if MD/MV, or 0
if not.

f1d3ff4564 Remove ata_removable() and ata_present() from multidriver code.
ata_removable() and ata_present() no longer exist (ATA drives are never
removable with our current drivers), so storage.c shouldn't try to call
them from the wrapper functions. This was never noticed because these
wrapper functions are only used for multidriver code, which happens not
to be used with ATA currently.

cc889e9d60 Change the thread api a bit.
* Remove THREAD_ID_CURRENT macro in favor of a thread_self() function, this allows thread functions to be simpler.
* thread_self_entry() shortcut for kernel.c.

acf98d9e7f fix broken case handling in storage_present() and storage_removable(). Those were buggy for targets with a hotswappable drive *and* more than one storage driver (i.e. only the D2 was probably affected)

376d8d577f Add IO priority handling. Currently all IO has equal priority, except the dircache scanning thread which is lower. This fixes the slow boot problem for me, with the added benefit that actual audio playback also starts faster.
Lots of the changes are due to changing storage_(read|write)sectors() from macros to wrapper functions. This means that they have to be called with IF_MD2(drive,) again.
