413da2a3d9 Rework PCM buffer 
* Linked list instead of static array buffer pointers
* Variable sized chunks
* Improved mix handling
* Reduction in duplicated code
* Reduced IRAM usage w/o sacrificing performance
* Converted to almost entirely unsigned math
* Add pause function to reduce pcm_* exposure to playback.

This WILL break playback on the iPod until linuxstb makes a followup commit.

1f39f2e748 Ported the memory guard debug feature to coldfire, using the breakpoint logic. There are 3 shortcomings compared to SH1: (1) While the setting itself survives RoLo, it cannot be read back because the debug module programming model is write only. (2) system_reboot() from a 'Debug' exception doesn't work because the CPU enters emulation mode, and the only way leaving this mode is via an rte instruction. (3) (fixable) Catching write accesses to the flash doesn't actually do anything because the flash memory area is set write protected in CSMR2, so a write attempt stalls the bus. * Cosmetic changes in system.c

cfb073c452 Coldfire: New timer handling on CPU frequency change, adjusting the prescaler on the fly, for both tick and user timer. Precondition is that the higher frequencies are integer multiples of the base: now NORMAL is 45 MHz and MAX is 124 MHz. Removes the need for applications with longer timer periods (>= 10 ms) to boost the CPU all the time, e.g. the grayscale lib. Timer counts are now always based on the base frequency (CPU_FREQ). * Adjusted the RAM refresh timers to the new frequencies (all frequencies for H100) * All: Fixed the tick timer count being off by one.

f090dc3829 Crossfade (and pcm buffer size) length is now configurable. 
Implemented anti-skip buffer setting for iriver also. Settings block
bumped up, PLEASE SAVE YOUR SETTINGS BEFORE UPGRADING.

e5d08722f8 Iriver: First attempt at recording. Use Info->Debug->PCM recording to test recording of wav-files. Seams to work fine except occasional 100 ms noise at pos 100 ms (not later) so initialization or synch problem..

4c7da88618 Iriver UDA1380 volume and balance handling is now done in sound.c, similar to archos player; removed pcm_set_volume(). Implemented a dB-linear scale. Sorted & bumped plugin api. Removed audio test from the debug menu.

a7f7781dca MMC driver does now handle block sizes != 512 bytes, which is necessary to support cards > 1 GB. Changed error handling to use the same method as other parts of rockbox, allowing to trace the call chain. Long policy, code cleanup.

2116bba296 New function for formatting large-range values for output, both printed and voiced. This replaces num2max5(). It is currently used for the total/free space display in the info menu, for the recorded number of bytes (recorders) and the MMC debug info (Ondios).

24c7c04899 adjustment for the Ondio: button driver has an Odio part, for now it has a Player layout. Some fixes in the app code were necessary to remove dependencies of LCD, keypad, this wasn't independent everywhere.

f09c5f4c63 Finally, merged the improved power code from Uwe Freese: 
- The battery level (percentage) is more realistic and considers if the charger is on.
- It considers the "lazyness" a battery shows when the charging has just turned on or off (see below). But this is not perfect by now.
- The battery level is good enough to estimate the remaining running time and the remaining charging time. And so the info screen now shows this info.
- The maximum time of a charging cycle is now dynamically calculated out of the battery level (percentage) and is not a fixed value.
- A minimum of 60 minutes is waited after a charging cycle stops before another one starts.
- Added another screen in the battery debug screen (press down three times).

b1b8bd4dd1 Moved on_screen, f2_screen, f3_screen and handle_usb (renamed usb_screen) to a new file: screens.c. 
typedef Menu replaced with a bool.
All code now calls usb_screen() for usb handling.
Nearly all code now deals with USB connect/disconnect properly.

b285076925 Remade the menu system slightly. All functions invoked from menus now use 
the Menu typedef as return type, and *ALL* menus that intercept USB connect
can then return MENU_REFRESH_DIR so that the parent (any parent really) that
do file or dir-accesses knows that and can do the refresh. If no refresh
is needed by the parent, MENU_OK is returned.

Somewhat biggish commit this close to 1.3, but we need to sort out this
refresh-after-usb-connected business.
