50a6ca39ad Move c/h files implementing/defining standard library stuff into a new libc directory, also standard'ify some parts of the code base (almost entirely #include fixes). 
This is to a) to cleanup firmware/common and firmware/include a bit, but also b) for Rockbox as an application which should use the host system's c library and headers, separating makes it easy to exclude our files from the build.

c61e89c0ed Make creat() posix compliant API-wise. Shouldn't affect the core as it's wrapped via a static inline.

499426caf9 remove MEM_FUNCTION_WRAPPERS, and private mem* implementations from plugins, and replace with pluginlib implementations in plugins/lib/gcc-support.c

23d9812273 loader-initialized global plugin API: 
struct plugin_api *rb is declared in PLUGIN_HEADER, and pointed to by
__header.api

the loader uses this pointer to initialize rb before calling entry_point

entry_point is no longer passed a pointer to the plugin API

all plugins, and pluginlib functions, are modified to refer to the
global rb

pluginlib functions which only served to copy the API pointer are
removed

2acc0ac542 Updated our source code header to explicitly mention that we are GPL v2 or 
later. We still need to hunt down snippets used that are not. 1324 modified
files...

26d242ae65 General housekeeping: Make plugin buffer functions take size_t * instead of int * to match the parameter type of the buffer functions called in the core. Get rid of unsafe int * <==> size_t * casting. Use ssize_t where int was used and size_t where unsigned int was used in the buffer calls to not alter signedness in the plugins. No API version change since it should only be an issue for 64-bit sim builds.

67eb154146 Removed 'mode' parameter from creat(). It wasn't pure posix anyway, it was ignored on target and mixed into 'oflags' in the simulator. * Simplified io.c a bit by defining a dummy O_BINARY for OSes which don't have that.

a36b1d4083 New plugin loader. Solves the crashes introduced with the .bss changes while keeping the small binary size. The model & api version check is now part of the plugin loader. Codecs are not yet adapted, but the old method still works for them. Simulator plugins are not (yet) version-checked. API version numbering restarted, as this is an all-new system. Uses the target ID from configure, so don't change that too often.

85e03767f7 Reorganized encoder to allow compressing blocks in smaller chunks and 
improved efficiency somewhat by looping through data in tighter passes.
Code is basically ready for an attempt at direct recording.

febd9750f1 Created new viewer application to test WavPack encoding. Accepts .wav file 
and generates lossless .wv with the same name. Runs at about 150% realtime
for now. Only problem is that folder display (and playlist?) do not get
updated when finished and since files don't show extensions it's a little
hard to tell which is which (wav is first by alpha).
